Facet joint syndrome occurs when the small joints at the back of the spine (facet joints) become inflamed, irritated, or arthritic. These joints allow the spine to flex, extend, and rotate — and when they degenerate, they can cause deep, aching pain in the neck or back that typically worsens with twisting, bending backward, or prolonged standing.
Facet joint pain is one of the most common causes of chronic spinal pain, particularly in older adults. It is often misdiagnosed as muscle strain because the pain patterns can overlap.
